Overview
The Ear, Nose and Throat [ENT] Department is abreast with advancement in medical technology and uses latest treatment modalities for ENT diseases while at the same time being sensitive towards patient care with a human touch. Many procedures are offered as DAY CARE, meaning the patient can walk in for the procedure in the morning and walk out of the hospital in the evening.
Ear Surgeries
Majority of the ear surgeries are done as day care procedures.
Myringotomy and Grommet insertion: It involves making an incision in the eardrum to drain middle ear fluid and putting in a ventilation tube. This is done under local anaesthesia without sedation in adult patients. Such patients may be sent home within 1 hour of the operation. Children are treated with short general or ketamine anaesthesia and sent home as soon as they are fully awake and are given the OK by the anaesthetist.
Tympanoplasty: Repair of the ear drum with restoration of the middle ear hearing apparatus. This is done under local anaesthesia with sedation and the patient is be sent home on the same day.
Mastoidectomy: This surgery is done for the treatment of diseases like cholesteatoma in the mastoid and middle ear. This may be done with or without reconstruction of the tympanic membrane and hearing apparatus. Cholesteatoma occurs as a complication of chronic ear infection of the middle ear. Although a major surgery, it can be done under local anaesthesia with sedation if the patient is comfortable. Otherwise general anaesthesia may be used. Although the surgery might take three hours or more, the patients can be sent home the same day. Few patients who have undergone mastoidectomy may experience vertigo. So they are counselled about it. If patients suffer from vertigo, they are advised a longer hospital stay.
Stapedectomy: This is a surgery for restoration of hearing in otosclerosis. This is usually done with local anaesthesia and sedation. These patients can sometimes have post-operative vertigo and even vomiting. In the event of vertigo, or complicated surgery, the patient stays in the hospital till the patient feels better. Most patients can however go home the same day. Air travel is not advisable immediately after this surgery.
Nasal Surgery
Most septal and sinus surgery are done under local anaesthesia with sedation. Nasal packing is done after many nasal surgeries. Patients are advised overnight hospital stay as a safety precaution, because in rare events there may be nasal bleeding.
Endoscopic Dacryocystorhinostomy(DCR) is done easily local anaesthesia and sedation. Patients can be sent home after nasal pack removal on the evening after surgery, provided there is no bleeding.
Endoscopic sinus surgery involving extensive polyposis or disease and in the posterior sinuses is done under general anaesthesia and these patients are usually advised to stay in hospital as there may be bleeding associated with major sinus surgery.
Endoscopic surgery for pituitary adenomas, optic nerve decompression, orbital surgery and CSF rhinorrhoea also involves staying in the hospital for a few days.
Patients who have epistaxis [medical term for nose bleed without any major cause] are ideally suited for day care endoscopic surgery. The bleeding points inside the nose can be identified and cauterized with the endoscope. Nose bleed cures are day care surgeries.
Throat and Laryngeal Surgeries
Tonsillectomy and adenoidectomy have always been done as day care procedures. The patients are sent home in the evening after adequate examination for bleeding.
Oral biopsies, buccal leucoplakia excision and such other minor surgeries are also day care procedures.
Even most rigid endoscopies, such as direct laryngoscopy, microlaryngoscopy and oesophagoscopy, done under general anaesthesia can also be done as day care cases.
Similarly, short procedures for diagnosis, biopsy, and therapeutic excision of small lesions or foreign body removal also do not need overnight stay.
Rigid bronchoscopy and surgery for tumours require prolonged hospital stay.
